REQUIRED DOCUMENTS (CHOOSE ONE DOCUMENT PER CATEGORY)
Proof of New York State Residency. (Any one of the following issued in your name):
- New York State DMV Identification (driver’s license, learner’s permit, or non-driver’s ID
- New York State Benefit card
- New York State Voter Registration card
- New York State Vehicle Registration
- New York State Apartment Lease
- New York Residential (Shelter) Program letter
- Postmarked mail with your name and your New York address
- Bank Statement including your name and your New York address
- US-issued refugee documentation (I97 Letter or I-797 application papers sent to you.
- Work authorization card.
- New York City- or New York State-issued temporary status card or letter with the name of the applicant
- New York City ID card issued in the applicant’s name
- If you live in New York but don’t have any of these documents, you’ll speak with a counselor to explore other options for proving New York residency.
Proof of 2024 Income (Any one of the following):
- Department of Labor ID card or letter
- 2024 Federal Tax Return (1040, 1040A, or 1040EZ).
- IRS Form 4506-T – filed by applicant or family with the IRS
- Copy of IRS Letter 1722 indicating that the student or parents who claim the student did not file a return for 2024.
- HRA Cash Assistance – Budget letter for 2024
- SSA/SSI Documentation – Social Security Award letter for 2024
- Veterans Administration Disability Benefits letter
- Unemployment, Pension, Annuity Benefits, or Workers’ Compensation letter for 2024
- Child Support or Alimony payment stubs for 2024. (Court order or affidavit)
- Residential (Shelter) Program Letter 2024
- An I97 or I797 acknowledgment letter or application
- US Work Authorization Card (red and white striped card) with any one of the following statuses: A03, A04, A05, A12, A13, A14, A16, C08, C11, C19, C20, C22, C33, C35, C36
- US asylum parole letter
- US asylum application
High School or GED diploma and/or transcript
- Required for applicants to Administrative Assisting, CNA, College Preparation, Electronic Health Records, EMT, Microsoft Essentials, and Pharmacy Tech.
- If you are applying to GED prep and have taken and passed any part of the GED or TASC or have taken and passed any New York State Regents Exams, bring your transcripts (unofficial is ok)!
